These Terms of Service ("Terms") describe the terms under which you may book and receive massage therapy services from Misty Malott, BS, LMT ("Misty," "I," or "the therapist"), operating as Aligned with Misty in affiliation with Tampa Bay Sports Massage and Therapy. By booking a session or receiving services, you agree to these Terms.
1. Services Provided
Services include sports massage therapy, deep tissue massage, therapeutic bodywork, injury recovery support, mobility and flexibility work, muscle tension relief, and performance recovery massage. All services are provided by a licensed massage therapist (Florida Massage License #MA 106918) and are intended to support recovery, mobility, and relief — not to replace medical care.
2. Not a Substitute for Medical Care
Massage therapy is a complementary wellness service. It is not a substitute for diagnosis, treatment, or advice from a licensed physician or other qualified medical provider. Misty does not diagnose illness, prescribe medication, or treat disease. If you have a medical condition, are pregnant, or are unsure whether massage is appropriate for you, consult your physician before booking.
3. Intake and Health Information
Before your first session, you will be asked to complete an intake form disclosing relevant health history, injuries, and concerns. It is your responsibility to provide accurate and complete information and to update Misty about any changes to your health. Withholding relevant information may affect the safety and effectiveness of your session.
4. Bookings, Cancellations, and No-Shows
Sessions are scheduled through the online booking system. Please arrive on time; late arrivals may result in a shortened session. Cancellations or rescheduling requests should be made at least 24 hours in advance. Cancellations within 24 hours of the appointment or failure to attend may be subject to a fee at the therapist's discretion.
5. Payment
Payment is due at the time of service unless otherwise arranged. Pricing and available session lengths are listed on the booking page. Prices are subject to change without notice.
6. Conduct and Consent
Massage therapy requires a professional, respectful environment. Misty reserves the right to decline, modify, or end a session at any time if a client's conduct is inappropriate, if consent is withdrawn, or if continuing would be unsafe. You may request changes to pressure, technique, or focus areas at any point during a session.
7. Results and Expectations
Outcomes vary from person to person. No specific result is guaranteed, and a single session may not resolve long-standing patterns. Recommendations for follow-up care or self-care are suggestions, not obligations.
8. Limitation of Liability
To the fullest extent permitted by law, Misty Malott and Tampa Bay Sports Massage and Therapy are not liable for any indirect, incidental, or consequential damages arising from services provided. Liability for direct damages is limited to the amount paid for the session in question.
9. Privacy
Your personal and health information is handled as described in the Privacy Policy. By booking, you consent to the collection and use of your information as outlined there.
10. Changes to These Terms
These Terms may be updated from time to time. The most current version will be posted on this page with an updated date.
